How to start home health care

  1. Call us or ask your doctor for a referral. Call 214-440-1394, or ask your doctor or hospital discharge planner to fax a referral to 214-440-1523.
  2. We check your coverage. Our team checks your insurance and confirms we serve your address. We work with your doctor on the orders.
  3. A nurse or therapist visits your home. A nurse or therapist comes to your home to start care and build your plan with you and your doctor.

Does Medicare cover home health care?

Yes. Medicare Part A and Part B cover home health care when all of these are true:

  • You are under the care of a doctor or other allowed practitioner who reviews your plan of care.
  • They certify that you need intermittent skilled nursing or therapy.
  • They certify that you are homebound.
  • The agency is Medicare-certified.

Do I need a doctor's referral?
Yes. Home health care starts with an order from a doctor, nurse practitioner, clinical nurse specialist, or physician assistant. Medicare also requires a face-to-face visit with your provider about the reason for home health. The visit must be within 90 days before or 30 days after care starts. If you do not have a referral yet, call us and we will work with your doctor.
